Of these 100 people, the 97 unhighlighted ones do not have the condition. Specificity asks what share of them the test correctly clears โ€” every mistake there becomes a false positive, and the 97 vastly outnumber the 3.

Example

Screening 10,000 people where 9,990 are healthy, a 99%-specific test wrongly flags about 100 of them โ€” ten times the 10 true cases it could possibly find.

How It Works

Specificity is scored only on the people who do not have the condition: of the 97 in this crowd without it, what share does the test correctly clear? A 99%-specific test wrongly flags 1 in 100 healthy people. That sounds negligible until you notice the healthy group is usually enormous: screen 10,000 people for something 1 in 1,000 have, and 1% of the 9,990 healthy people is about 100 false positives โ€” swamping the roughly 10 true cases even with 99% . Sensitivity guards against missing cases; specificity guards against crying wolf.
